Registration
To register with this practice, please complete the change of circumstances part of your medical card, sign it and bring or send to us.  If you cannot find your medical card, we can give you a form to complete.  Patient care is shared in this practice and any of the doctors will be happy to see you.  Please tell us of any change in your name, address or telephone number so that we are able to keep you records accurate and contact you if this is needed.
If you are a Carer of another person, or if you have a Carer, please let us know.  You will be given a copy of our Policy and Protocol for Carers and will be asked to sign a consent form so that the information can be entered on the Carer and the Cared for person’s computer record.

Private Fees
Some services that the practice provide are not covered by the NHS and a fee will be charged. View a list of these charges by clicking here
